Privacy Statement

Our commitment to your privacy:

We are committed to handling personal information about you, including health information about you, in accordance with the requirements of the Commonwealth Privacy Act 1988.

  • what kind of information we collect and hold about you
  • how and why we collect it
  • what we do with that information and who we share it with (and when)
  • your right to seek access to, and if required correction of, the records we hold about you
  • your right to make a privacy complaint, to us and others
  • whether we are likely to disclose information about you to overseas recipients.

General:

  • We will only use your personal information for the purpose of your personal medical care, unless you have consented to us using your personal information for other purposes; or its use for another purpose is required or permitted by law.
  • We will assume that any personal information provided by you is free from errors and omissions, is not misleading or deceptive and complies with all relevant laws.
  • The Physiotherapist may need to seek information from a hospital or your doctor to properly assess your medical condition,  form an opinion about your condition and about further desirable tests and treatment.  This only applies to information relevant to your current condition.
  • We may also need to write to your doctor, or sometimes under the requirements of law (e.g. Return to Work Act) to your employer or your solicitor to detail our opinion.

Release of medical information:

  • Olympic Sports Clinic will not release your medical file, unless required to do so by Law.
  • In compliance with the Privacy Act, Olympic Sports Clinic will allow you to have access to your file. This will be done with a staff member to address any questions you have and to explain entries in the medical file.  You will need to make an appointment for this, complete an application form, produce photo identification, indicate which aspect of your file you wish to examine and an administrative charge may apply.
